This department is the central storekeeper of all our patients’ medical records.

Every time a patient receives healthcare at JPMC, a record file is created to maintain the observations, medical or surgical interventions, and treatment outcomes.

This record also includes information that the patient provides concerning his or her symptoms and medical history, the results of examinations, reports of x-rays and laboratory tests, diagnoses and treatment plans. Medical records and health information technicians organise and evaluate these records for completeness and accuracy.

Patients can also request for a copy of their medical information through this department or through their doctor.

 

Release of Medical Information

The Medical Records Department is responsible to fulfil all medical record requests for medical information. It is our policy to charge a nominal fee for all copies except those sent directly from JPMC to another health care facility or organisation.

 

Private / Personal Request

To request for your medical records, you may either call or come down personally to the medical centre to request for your medical records from the clinic you visited or from your doctor or directly from the Medical Records Department. This request will then be processed and upon approval from your doctor, your medical information will be released to you within 3 to 5 working days. You will receive a call when your records are ready for collection.

 

Personal Request for Insurance Company

Patients can request for their records for their insurance company. JPMC requires a letter of consent from the patient for the release of information to their insurance company. The requestor is responsible for copy charges.

 

Corporate Request

Private organisations wishing to request for a staff’s medical records are requested to call the Medical Records Department for further details and arrangements.

 

Collection

You must personally collect your medical records, as you would be required to sign a Release of Information (ROI) Form and make the necessary payments upon collection. Please remember to bring along your identification card or passport for security purposes.
